Passport Applications

Sandi Caulkins is an authorized passport application acceptance agent for the U.S. Department of State.

**By Appointment only:  during library hours**

Fees for application submission are set by the U.S. Department of State.

Routine Service --- Receipt within 6 weeks --- These Fees are Non-Refundable

Expedite Service Fees: See above link for most current information

Passport Photo Fee:  $10

Photos are also taken for any form of ID such as Concealed Handgun, ID cards, etc.

Passport Renewal:  Customer Submits                                   

On Renewals we only take the new photo ($10) and provide the Form DS-82 Applicant completes & mails.
